Finding a doula who is genuinely affirming, not just checking a box, can feel like a lot of work on top of everything else that comes with pregnancy and birth. Here's what to actually look for.
Affirming Means More Than a Rainbow Flag
A truly affirming doula has done real work to understand the experiences of LGBTQ+ birthing people and families. That means using correct pronouns and names without being asked twice, understanding the unique dynamics of different family structures, and knowing how to navigate birth spaces that aren't always as welcoming as they should be.
They Don't Make Assumptions
An affirming doula won't assume your relationship structure, your gender identity, or what language feels right for your body. They'll ask, listen, and follow your lead. If a doula uses gendered language without checking in during your consultation, that's worth paying attention to.
They Can Advocate for You in the Room
Birth settings can be inconsistent when it comes to LGBTQ+ affirming care. A good affirming doula knows how to step in when providers use the wrong language, make assumptions, or create an unwelcoming environment, without making the situation more stressful for you.
They Understand Your Unique Journey
Whether you conceived through IVF, IUI, surrogacy, or adoption, whether you have one parent, two, or more, an affirming doula gets that your path to parenthood may look different and that's completely fine. They're there to support your birth, not to fit you into a standard template.
They Walk the Talk
Look for doulas who are actively engaged in LGBTQ+ affirming birthwork, not just those who list it as a service. Do they mention it on their website in a genuine way? Do they have experience supporting queer and trans families? Have they done training around gender affirming care?
